NOMINATION FORM

*Coaches must be members of ICGSA in order for their players to be nominated.

*Nominations must be returned no later than Saturday, October 20, 2014.  All nominations must be mailed or emailed to Phil Slavens.

*All ICGSA head coaches may participate in the voting for All State selection. The selection will take place, October 27th, immediately after the Class A championship game scheduled for 1:00pm. 

*All active soccer ICGSA coaches are invited to participate in the All-State voting process. Food and drinks will be provided at IUPUI Stadium.

*Due to the need to generate ballots in advance of the selections at the State Finals, late ballots cannot be accepted. This includes walk-in nominations at Indianapolis.
