2012 NORTH/SOUTH ALL-STAR NOMINATION FORM
The tennis coach must be a current member of ICGSA to nominate a player. Coach’s membership may accompany nomination form. Include membership payment with coach’s form. www.icgsa.org
All information must be typed or printed neatly or it will not be accepted. Information will be placed in the All-Star program on June 10, 2012 and sent to area newspapers.
Senior players will play a modified format of tennis during the day. Three rounds of tennis will be played by all players consisting of singles and/or doubles and a tie breaker set to showcase all players. Coaches of the tennis cup will be the ICGSA District Coach of the Year tennis coaches throughout Indiana. Only the designated high school tennis coach may nominate Senior varsity players. Information will be from the high school season only. Information about wins and losses against other 2012 nominees is vitally important to proper selection. Coaches and players will be notified by June 1, 2012 for selection to All-Star team.
Coaches are required to provide a complete listing, typed or printed neatly, of the nominee’s 2012 tennis season only. Include position played, opponent, school, date, and score for each match.

Any updates to the senior record once selected to the All Star Tennis team must be received no later than June 4, 2012 to be included in the program (tournament updates will be added in by the tennis committee).
If you nominate more than one player, duplicate this form. The tennis committee will e-mail or call your player directly if selected to the team. If no information is given than the school and/or tennis coach will be called and it will be up to the tennis coach to inform their player of the recognition and all deadlines.
Nominations are for SENIOR varsity players only, singles and/or doubles. This is a senior honor. The player nominated should be your schools player that has a winning record during the season. Tournament info can be added by the coach and tennis committee.
