If
you are unable to locate a form on this page, please check the specific
sport page (i.e. basketball, softball, tennis, etc.) for sport-specific
form listings.
Please remember that memberships are good
for the school year regardless of when it is paid throughout the year. In
order to receive all mailings, pay your membership early. Members at the
end of the previous year will receive the August mailings but will only
receive mailings after August if their membership has been renewed.
NOTICE: Each link/form printed from this page should be no more than one page.
If the form prints on multiple pages:
Decrease your font size when using Navigator.
Use the "Smaller" or "Smallest" settings under "Fonts" when using Internet Explorer.
Click on the link(s) of the form(s) you need and print the page to your local printer.
ADDRESS CHANGE:
If your address changes during the year, please e-mail changes to make sure that you continue to receive all ICGSA information.
WARNING: When filling out ICGSA forms, be
certain that all information is complete and instructions have been followed. Carefully check your work; not doing so may result in your form not being accepted. If you are using any form of technology, be sure to follow-up on your work. Due to server problems, power outages, slow mail service, etc, your information may not be received.
